About the Role

Key Responsibilities

• Conduct comprehensive virtual assessments, including history taking and physical exam guidance via video.
• Develop and implement individualized care plans, prescribing medications and ordering labs as appropriate.
• Monitor chronic disease progression (e.g., diabetes, hypertension, COPD) and adjust treatment protocols.
• Provide patient education on lifestyle modifications, medication adherence, and preventive screenings.
• Coordinate referrals to specialists and facilitate seamless transitions of care.

Requirements

Qualifications

• Active, unrestricted Nurse Practitioner license in at least one U.S. state; multi‑state licensure preferred.
• Master’s or Doctoral degree in Nursing (MSN, DNP) from an accredited program.
• Board certification (FNP‑BC, AGPCNP‑BC, PMHNP‑BC, or equivalent) through AANP or ANCC.

Salary Range
