Key Responsibilities

• Serve as the primary point of contact for patients, assessing needs, and developing individualized care plans tailored to specific health goals.
• Coordinate appointments, treatments, and follow-ups between patients, providers, and insurance partners, minimizing delays and miscommunication.
• Monitor patient progress through electronic health records (EHR) and telehealth platforms, adjusting care strategies to optimize health outcomes.
• Maintain accurate medical documentation and ensure strict compliance with HIPAA, Joint Commission, and state regulatory standards.
• Facilitate seamless communication between multidisciplinary healthcare teams, including nurses, social workers, pharmacists, and specialists.

• Generate and analyze care coordination metrics to identify trends, improve processes, and report performance to leadership.

Requirements

Candidates must possess a minimum of 2–3 years of experience in care coordination, patient navigation, or a related healthcare role. A bachelor’s degree in Nursing, Health Administration, Public Health, or a closely related field is preferred but not mandatory for applicants with substantial hands-on experience. Essential qualifications include proficiency with EHR systems (such as Epic, Cerner, or Athenahealth), strong written and verbal communication skills, and the ability to thrive in a fast-paced remote environment. Salary & Compensation
