Fidelis Care is hiring a Senior LTSS Assessor to perform comprehensive care management and long‑term services and supports (LTSS) assessments for members in Bushwick and Williamsburg, Brooklyn. Key Responsibilities

• Conduct UAS (Unified Assessment System) assessments for eligible members residing in Brooklyn or the Bronx.
• Evaluate medical, functional, and social needs to develop individualized care plans.
• Coordinate with interdisciplinary teams, including physicians, nurses, social workers, and community resources.
• Monitor progress, adjust care plans, and ensure compliance with federal and state regulations.
• Maintain accurate documentation in electronic health record systems.
• Provide education and support to members and their families regarding LTSS options.

Qualifications

• Bachelor’s degree in Nursing, Social Work, Public Health, or related field; Master’s preferred.
• Minimum 5 years experience in long‑term services and supports assessment or case management.
• Current NY State licensure (RN, LCSW, or equivalent) or relevant certification.
• Strong knowledge of Medicaid, Medicare, and community‑based services.
• Excellent communication, analytical, and problem‑solving skills.
• Ability to travel within Brooklyn and the Bronx for on‑site assessments.
