Key Responsibilities
- Supervise and mentor a team of case managers, therapists, and support staff delivering CLTS services.
- Ensure compliance with Medicaid, state, and federal regulations for the CLTS waiver program.
- Develop individualized service plans, monitor progress, and adjust interventions to meet each child’s developmental, physical, or emotional needs.
- Coordinate with families, schools, healthcare providers, and community partners to secure comprehensive supports.
- Prepare and present program reports, budgets, and performance metrics to senior leadership.
- Maintain accurate documentation in the agency’s electronic health record system.

Qualifications
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field (Master’s preferred).
- Minimum 3 years of experience working with children with developmental, physical, or severe emotional disabilities, preferably in a Medicaid waiver setting.
- Prior supervisory or team‑lead experience.
- Strong knowledge of Wisconsin Medicaid policies and CLTS waiver requirements.

- Valid Wisconsin driver’s license and reliable transportation.
