Job Summary
UnitedHealth Group is seeking a Medical Director – Post‑Acute Care Management – Care Transitions to work remotely from Boston, Suffolk County. This senior clinical leadership role drives strategy and quality improvement for post‑acute care services.
Key Responsibilities
- Lead the design, implementation, and evaluation of post‑acute care transition programs across Optum’s network.
- Develop clinical protocols, pathways, and performance metrics to improve patient outcomes and reduce readmissions.
- Collaborate with physicians, nurses, case managers, and external partners to ensure seamless care coordination.
- Provide expert clinical guidance on complex cases and serve as a mentor to multidisciplinary teams.
- Analyze data and health‑information systems to identify trends, report outcomes, and inform continuous improvement.
- Ensure compliance with regulatory standards, accreditation requirements, and UnitedHealth Group policies.
- Represent the organization in external forums, conferences, and stakeholder meetings.
Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) with active, unrestricted medical license.
- Board certification in Internal Medicine, Family Medicine, or a related specialty.
- Minimum 8 years of clinical experience, including at least 3 years in post‑acute care, care transitions, or population health management.
- Proven leadership experience overseeing multidisciplinary teams and large‑scale care programs.
- Strong analytical skills with proficiency in data‑driven decision making and healthcare analytics tools.
- Excellent communication, presentation, and stakeholder‑engagement abilities.
- Ability to work independently in a remote environment while maintaining alignment with corporate goals.
Compensation & Benefits
