Trinity Health is hiring a full‑time Nursing Home Medical Director/Physician to lead clinical care at The Eddy, a nonprofit skilled nursing facility in the Capital Region of Upstate New York. Salary: $117,514.77 per annum.

Key Responsibilities

• Provide on‑site medical leadership and direct patient care for residents in a skilled nursing home setting.
• Develop, implement and monitor clinical protocols, quality‑improvement initiatives and regulatory compliance.
• Collaborate with interdisciplinary teams – nursing, therapy, social work and administration – to deliver person‑centred geriatric care.
• Supervise and mentor resident physicians, nurse practitioners and other clinical staff.
• Conduct regular chart reviews, case conferences and resident assessments.

Essential Requirements

• Medical degree (MD or DO) with board certification or eligibility in Geriatric Medicine or Family Medicine.
