Key Responsibilities

• Coordinate patient enrollment and ongoing support for programs such as CCM, PCM, RPM, RTM, APCM, TCM, BHI, CHI, and PIN.
• Facilitate smooth care transitions by communicating with clinicians, patients, and external partners.
• Monitor patient data, ensure compliance with regulatory standards, and flag any clinical concerns.
• Provide education and resources to patients for chronic condition management, behavioral health, and preventive care.
• Document all interactions accurately in the EMR and generate regular performance reports.
• Collaborate with the operations team to streamline workflows and improve program efficiency.

Qualifications

• High school diploma or GED required; Associate’s or Bachelor’s degree in health administration, nursing, or related field preferred.
• 2+ years of experience in care coordination, case management, or patient services.
• Strong knowledge of chronic care models (CCM, RPM, TCM, etc.) and relevant compliance guidelines.
