Position Overview

BrightPath Health Services is expanding its remote care network and seeks a dedicated Direct Support Professional to provide high‑quality, person‑centered assistance to clients across California. Key Responsibilities

Core Duties

Conduct virtual check‑ins and assessments to evaluate client needs and adjust support plans in real time, ensuring timely interventions.

Develop and maintain digital documentation, including progress notes, treatment plans, and compliance reports using secure, HIPAA‑compliant cloud‑based systems.

Facilitate telehealth sessions, coordinating with physicians, therapists, and family members to align care strategies and monitor health metrics.

Qualifications & Requirements

What We’re Looking For

Minimum two years of experience in direct support, case management, or a related health‑care field, preferably in a remote or tele‑health environment.

Strong interpersonal skills, empathy, and the ability to build trusting relationships with clients and families through virtual communication.
