Position Overview

BrightPath Health Services is expanding its remote care network and seeks a dedicated Direct Support Professional to provide high‑quality, person‑centered assistance to clients across California. In this fully remote capacity, you will coordinate daily activities, monitor progress, and ensure each individual’s health, safety, and personal goals are met, all from the convenience of your home office. You will be an integral part of a multidisciplinary team that empowers clients to achieve greater independence and community participation.

Key Responsibilities

Core Duties

Conduct virtual check‑ins and assessments to evaluate client needs and adjust support plans in real time, ensuring timely interventions.

Develop and maintain digital documentation, including progress notes, treatment plans, and compliance reports using secure, HIPAA‑compliant cloud‑based systems.

Facilitate telehealth sessions, coordinating with physicians, therapists, and family members to align care strategies and monitor health metrics.

Provide crisis intervention and emotional support through phone, video, or chat, ensuring immediate safety for clients and their households.

Collaborate with a multidisciplinary team, attending virtual case conferences, sharing best practices, and contributing to continuous quality improvement initiatives.

Analyze outcome data to identify trends, measure effectiveness of interventions, and report findings to leadership for strategic planning.

Qualifications & Requirements

What We’re Looking For

Minimum two years of experience in direct support, case management, or a related health‑care field, preferably in a remote or tele‑health environment.

Valid California driver’s license (for occasional in‑person visits) or ability to travel regionally as needed; however, the primary work location is fully remote.

Strong interpersonal skills, empathy, and the ability to build trusting relationships with clients and families through virtual communication.

Proficiency with electronic health record (EHR) platforms, video conferencing tools, and digital documentation software such as Zoom, Microsoft Teams, and Epic.

Certified in CPR, First Aid, and any relevant state‑specific support certifications; a Bachelor’s degree in Social Work, Psychology, or a related discipline is preferred.

Compensation

Salary Range
