Responsibilities

Conduct virtual patient assessments and monitoring via telehealth platforms. Develop and implement individualized care plans for post-surgical and chronic condition management. Provide education on medications, wound care, and recovery protocols. Collaborate with healthcare teams to ensure continuity of care. Document patient interactions accurately in the electronic health record system.

Requirements

Education and Experience

Active RN license in good standing. Bachelor of Science in Nursing (BSN) preferred. Minimum 2 years of experience in medical/surgical nursing. Previous telehealth or virtual care experience highly desirable. Basic Life Support (BLS) certification required.

Skills and Qualifications
