Join a forward-thinking healthcare team delivering high-acuity virtual care to patients across Texas. We are seeking a compassionate, tech-savvy Registered Nurse to manage complex caseloads, drive clinical outcomes, and enjoy true work-life balance from your home office.

About the Role

As a Remote Registered Nurse, you will serve as the primary clinical point of contact for a diverse patient panel utilizing our proprietary telehealth platform. This is not a triage line; it is a longitudinal care management position where you build lasting relationships, coordinate with interdisciplinary teams, and practice at the top of your license. You will leverage advanced remote patient monitoring (RPM) tools, electronic health records (EHR), and video conferencing to assess, educate, and advocate for patients managing chronic conditions such as CHF, COPD, diabetes, and post-acute transitions.

We prioritize clinical autonomy. You will have the authority to adjust care plans within established protocols, initiate telehealth visits for early intervention, and collaborate directly with physicians, pharmacists, and social workers. This role eliminates the physical toll of bedside nursing—no heavy lifting, no 12-hour shifts on your feet, and no exposure to workplace violence—while amplifying your clinical impact through technology.

Key Responsibilities

  • Conduct comprehensive virtual health assessments and medication reconciliations for assigned patient panels.
  • Develop and implement individualized care plans utilizing evidence-based clinical guidelines and RPM data analytics.
  • Provide high-touch health coaching and disease management education via video, phone, and secure messaging.
  • Monitor real-time biometric alerts (blood pressure, glucose, weight, SpO2) and execute rapid clinical interventions.
  • Coordinate seamless transitions of care between hospital, home health, specialty providers, and community resources.
  • Maintain impeccable documentation in the EHR (Epic experience preferred) ensuring regulatory compliance and quality metrics.
  • Participate in weekly interdisciplinary team huddles and monthly quality improvement initiatives.

Requirements & Qualifications

Must-Haves

  • Active, Unencumbered RN License in Texas (Compact license accepted if primary residence is in Texas).
  • Minimum 3 years of acute care experience (Med-Surg, ICU, ER, Step-Down, or Telemetry strongly preferred).
  • Proven care management or utilization review experience (1+ year) or strong discharge planning background.
  • Proficiency with EHR systems (Epic, Cerner, or Athenahealth) and Microsoft Office Suite.
  • Reliable high-speed internet connection and a private, HIPAA-compliant home workspace.
  • Current BLS Certification (AHA); ACLS preferred.

Nice-to-Haves

  • Certification in Case Management (CCM) or Ambulatory Care Nursing (RN-BC).
  • Experience with Remote Patient Monitoring platforms (e.g., Current Health, Biofourmis, Vivify).
  • Bilingual proficiency in English/Spanish.
  • Previous work-from-home or telehealth experience.

Salary Range & Compensation

Estimated Annual Salary: $85,000 – $105,000 USD/year (Base + Performance Bonuses).

Compensation is determined by years of relevant experience, certifications held, and specific clinical competencies. We offer a transparent pay structure with annual merit reviews and quarterly bonuses tied to patient satisfaction scores, quality metrics (HEDIS/Stars), and engagement targets. This is a full-time, exempt (salaried) position with a predictable Monday–Friday schedule (core hours 9 AM – 5 PM CT) and zero mandatory weekends or holidays.

Our Culture: Clinical Excellence Meets Flexibility

We are a clinician-founded organization. Our leadership team includes former bedside nurses who understand the burnout cycle. We have engineered the “noise” out of nursing—no floating, no mandatory overtime, no charting on weekends. You will join a collaborative, high-trust team that communicates via Slack huddles, virtual coffee breaks, and quarterly in-person retreats (travel covered) in Austin or Dallas. We measure success by patient activation measures (PAM scores) and nurse retention rates, not just RVUs.

How We Hire

Our process respects your time and clinical expertise:

  1. Application Review: Our clinical recruiters screen resumes within 48 hours.
  2. Phone Screen (20 mins): Discuss your clinical background, remote setup, and career goals.
  3. Virtual Panel Interview (45 mins): Meet the Hiring Manager and a Senior Peer RN. Scenario-based clinical questions only—no “greatest weakness” questions.
  4. Skills Assessment: Brief EHR documentation simulation and RPM dashboard review.
  5. Offer & Onboarding: Fully remote onboarding program spanning two weeks with a dedicated preceptor.

Ready to Practice Nursing Differently?

If you hold an active Texas RN license, crave clinical autonomy, and want to ditch the commute without ditching the complexity of patient care, this is your next career move. We are building the future of healthcare delivery—one virtual visit at a time.
