Salary Range

Key Responsibilities

Virtual Client Support

You will conduct daily or weekly wellness calls to check on the physical and mental status of clients. This includes documenting any changes in condition, providing friendly companionship to combat isolation, and offering gentle reminders for appointments or medications using our secure internal software.

Care Coordination

Act as the primary point of contact for coordinating in-home services (if applicable in the client’s area), managing schedules, and communicating updates to family members and healthcare professionals. Your organizational skills will ensure that every client receives seamless, uninterrupted care.

Documentation & Reporting

Accuracy is key in healthcare. You will be responsible for updating client files, logging call notes, and flagging any urgent concerns to the supervising nurse or care manager immediately. Proficiency with digital charting systems is a plus.

Requirements

• High School Diploma or equivalent; CNA or HHA certification is highly preferred.
• Proven experience in caregiving, personal care, or a related healthcare support role.
