About the Role

As a Remote Home Health Aide with CareBridge Health, you will provide essential daily living assistance to patients in their own residences. Your responsibilities include monitoring vital signs, assisting with mobility, medication reminders, personal hygiene, and light housekeeping. You will document care activities in our secure electronic health record system and communicate regularly with supervising nurses and family members to ensure care plans are followed precisely. The position offers the autonomy to manage your caseload while receiving continuous support from a dedicated clinical team.

Key Responsibilities

• Deliver hands‑on personal care and companionship to assigned clients.
• Accurately record vital signs, medication administration, and care notes in real time.
• Collaborate with RN supervisors to adjust care plans based on client progress.
• Maintain strict adherence to HIPAA, OSHA, and state‑specific home‑care regulations.
• Participate in weekly virtual huddles and ongoing competency training.

Requirements

Education & Certification

• High school diploma or equivalent; completion of a state‑approved Home Health Aide training program.
• Current Certified Nursing Assistant (CNA) or Home Health Aide certification in at least one U.S. state.

Experience

• Minimum 1 year of direct patient care experience in home health, assisted living, or hospital setting.

Salary Range

$42,000 – $58,000 USD per year, commensurate with experience, certifications, and geographic differentials. Performance‑based bonuses and shift differentials are available for evening, weekend, and holiday coverage.
