Key Responsibilities

• Conduct regular virtual wellness checks and provide emotional support to clients and their families.
• Coordinate and schedule medical appointments, therapy sessions, and social activities via digital platforms.
• Maintain meticulous digital records and documentation in compliance with state and federal healthcare regulations.
• Advocate for clients’ rights and needs during virtual multidisciplinary team meetings.
• Develop and monitor individualized service plans to ensure goals are being met and adjusted in real-time.
• Collaborate with on-site caregivers to ensure a seamless continuum of care.

Salary Range

$42,000 – $58,000 USD/year

Salary is commensurate with experience and certifications. Competitive bonuses are available for those with specialized certifications in behavioral health or case management.

Requirements

Qualifications

• Experience: At least 2 years of experience in social work, caregiving, behavioral health, or a similar support role.
