Key Responsibilities

Client Care & Support

• Assist clients with fundamental everyday tasks including personal hygiene, nutrition, grooming, and medication management
• Conduct thorough needs assessments to identify specific support requirements and barriers to independence
• Develop and implement customized care plans in collaboration with medical professionals and family members

Documentation & Communication

Maintain detailed documentation of client interactions, progress notes, and care decisions. Coordinate with other healthcare providers to ensure comprehensive, holistic care coordination and maintain clear lines of communication with families.

Qualifications & Experience

Required Attributes

While prior experience in healthcare, social work, or community support services is advantageous, we actively seek motivated individuals with strong interpersonal skills. Candidates should demonstrate proven ability to build rapport quickly, adapt to diverse situations, and remain patient under pressure. Previous roles as home health aides, case managers, or volunteer caregivers are particularly valued. Salary Range

Our competitive salary range for this position starts at $45,000 – $65,000 USD per year and can increase with additional experience and achievements.
