About the Role

Our organization is seeking a dedicated Direct Support Professional to provide high‑quality, person‑centered support to clients across the country. Working remotely, you will collaborate with families, caregivers, and multidisciplinary teams to design and implement individualized support plans. This role blends virtual coordination with occasional in‑person visits, ensuring clients receive consistent, respectful, and empowering assistance.

Key Responsibilities

Provide remote case management and daily living skill coaching through video calls, phone sessions, and secure messaging platforms. Develop, monitor, and adjust personalized support plans based on client goals and progress. Coordinate with healthcare providers, educators, and community resources to facilitate seamless service integration. Document all interactions and outcomes in compliance with industry standards and confidentiality protocols. Advocate for client rights and promote inclusive practices within the organization.

Requirements & Qualifications

Minimum of 2 years of experience in disability support, home health, or related human services. Strong interpersonal skills with the ability to build trust and rapport virtually. Proficiency with digital communication tools (Zoom, Microsoft Teams, electronic health records). Bachelor’s degree in Social Work, Psychology, or a related field is preferred, but equivalent experience will be considered. Valid driver’s license and reliable internet connection are required. Background check clearance and eligibility to work in the United States.
