Fidelis Care is hiring a Senior LTSS Assessor (RN) in Latham, Albany County, US. This full‑time field role supports our Long‑Term Services and Supports (LTSS) members with a competitive annual salary of $140,508.

Key Responsibilities

• Conduct comprehensive clinical assessments for LTSS members in the Albany and Rensselaer areas.
• Develop, coordinate, and monitor individualized care plans in collaboration with interdisciplinary teams.
• Utilize Utilization Authorization System (UAS) and case management best practices to ensure appropriate service delivery.
• Document assessments, referrals, and outcomes accurately in electronic health records.

• Maintain compliance with state and federal regulations, including Medicaid and Medicare guidelines.
