About This Position
CareHarmony is seeking a Remote Triage Nurse (LPN or RN) to work night shifts full‑time, providing expert clinical guidance to patients and caregivers across the United States.
Key Responsibilities
- Conduct telephone and video triage for patients presenting with acute or chronic health concerns.
- Assess symptoms, prioritize urgency, and provide evidence‑based recommendations or referrals.
- Document all interactions accurately in the electronic health record (EHR) in compliance with HIPAA.
- Collaborate with the Lead Triage RN and multidisciplinary care teams to ensure continuity of care.
- Educate patients on self‑management strategies, medication adherence, and when to seek higher‑level care.
- Participate in quality‑improvement initiatives and contribute to value‑based care metrics.
Required Qualifications
- Current active LPN or RN license in any US state.
- Minimum 2 years of clinical experience in acute care, urgent care, or telehealth.
- Strong clinical assessment, critical thinking, and communication skills.
- Proficiency with EHR systems and comfortable using video conferencing tools.
- Reliable high‑speed internet connection and a private, HIPAA‑compliant workspace.
Preferred Skills
