Job Summary

Key Responsibilities
- Provide direct patient care in the primary care clinic, including assessments, medication administration, wound care, and health education.
- Collaborate closely with physicians, nurse practitioners, and multidisciplinary team members to develop and implement individualized care plans.
- Serve as an Electronic Medical Record (EMR) champion: train staff, troubleshoot issues, and ensure accurate documentation.

Qualifications

- Minimum 1‑2 years of primary care or outpatient nursing experience.

- Strong communication, interpersonal, and patient‑centered care skills.
- Ability to work both in‑person and via telehealth platforms.
