Job Summary
UnitedHealth Group is seeking a Medical Director – Post‑Acute Care Management – Care Transitions to join our Optum team. This remote role serves patients across the United States while being based in Boston, MA.
Key Responsibilities
- Lead clinical strategy and oversight for post‑acute care management and care transition programs.
- Collaborate with multidisciplinary teams to develop evidence‑based protocols that improve patient outcomes and reduce readmissions.
- Analyze data and health‑care metrics to identify trends, drive quality improvement, and ensure compliance with regulatory standards.
- Provide mentorship and guidance to physicians, nurses, and allied health professionals within the care transition network.
- Represent UnitedHealth Group in external stakeholder meetings, including hospitals, skilled‑nursing facilities, and payer organizations.
- Contribute to the development of technology solutions that support care coordination and patient engagement.
Qualifications
- Board‑certified or eligible physician with a medical degree (MD or DO).
- Minimum 5 years of clinical experience in post‑acute care, care transitions, or related fields.
- Demonstrated leadership experience managing clinical programs or teams.
- Strong analytical skills with proficiency in interpreting clinical data and health‑care analytics.
- Excellent communication and interpersonal abilities to influence cross‑functional partners.
- Eligibility to work in the United States.
Compensation & Benefits
Base salary: $113,366.91 annually. Comprehensive benefits package includes medical, dental, vision, 401(k) matching, tuition assistance, and generous paid time off.
