Job Summary
Carolina Complete Health is seeking an experienced LTSS Service Care Manager in Sidestown, Cabarrus County, US. This role supports 28 million members by coordinating holistic, cost‑effective care plans.
Key Responsibilities
- Assist in developing, assessing, and implementing personalized long‑term services and supports (LTSS) care plans.
- Coordinate multidisciplinary teams to ensure quality outcomes and adherence to regulatory standards.
- Monitor member progress, adjust care strategies, and document outcomes in the electronic health record.
- Analyze cost‑effectiveness of services and recommend improvements to reduce waste.

Compensation & Benefits
