About This Position

Join Centene as a LTSS Service Care Coordinator and help improve the health of 28 million members across the United States. This remote‑first role requires 50%‑75% local travel within Iowa for field visits while the remainder of the work can be done from home.

Key Responsibilities

  • Coordinate long‑term services and supports (LTSS) for members, ensuring timely access to care and resources.
  • Conduct on‑site assessments and visits in Iowa, building strong relationships with members, families, and community partners.
  • Develop individualized care plans, monitor progress, and adjust services to meet evolving needs.

Qualifications

  • 2+ years of experience in care coordination, case management, or community health.
  • Strong communication and interpersonal skills; ability to work independently and travel frequently within Iowa.
  • Proficiency with electronic health record (EHR) systems and Microsoft Office Suite.
  • Valid driver’s license and reliable transportation for field visits.
