Job Summary
Centene Corporation is seeking an LTSS Service Care Coordinator in Ames, Story County, Iowa. This role supports our 28 million members by coordinating long‑term services and supports (LTSS) while balancing remote work and extensive local travel.
Key Responsibilities
- Develop and manage individualized care plans for LTSS members, ensuring alignment with clinical and social service goals.
- Conduct 50%–75% on‑site visits across Iowa to assess member needs, coordinate resources, and build community partnerships.
- Collaborate with internal care teams, external providers, and family caregivers to facilitate seamless service delivery.
- Maintain accurate documentation in Centene’s health information systems and track performance metrics.
- Identify gaps in services, propose improvements, and contribute to continuous quality initiatives.
- Provide remote support, virtual consultations, and follow‑up communications when off‑site.
Qualifications
- Residency in Iowa (required for travel obligations).
- Bachelor’s degree in Social Work, Nursing, Public Health, or related field; Master’s preferred.
- Minimum 2 years experience in LTSS, case management, or community health coordination.
- Strong knowledge of Medicaid/Medicare LTSS programs and state regulations.
- Excellent interpersonal, communication, and problem‑solving skills.
- Proficiency with electronic health record (EHR) systems and Microsoft Office Suite.
- Valid driver’s license and ability to travel frequently within the state.
Compensation & Benefits
