Key Responsibilities

• Coordinate long‑term services and supports (LTSS) for members, ensuring timely access to care and resources.
• Conduct in‑person assessments and home visits in Champaign and Vermilion counties, traveling up to 50% of the time.
• Develop individualized care plans in collaboration with members, families, and interdisciplinary teams.

Qualifications

• 2+ years of experience coordinating LTSS or similar health‑care services.
• Strong knowledge of Illinois Medicaid and Medicare regulations.
• Excellent communication, problem‑solving, and interpersonal skills.
• Valid driver’s license and reliable vehicle for travel within Champaign and Vermilion counties.
• Ability to work independently in a remote setting while meeting travel requirements.
