Job Summary
The LTSS Service Care Coordinator at Meridian Illinois supports aging and physically disabled members across Champaign and Vermilion counties. This remote, work‑from‑home role includes up to 50% travel within the local area.
Key Responsibilities
- Coordinate long‑term services and supports (LTSS) for members, ensuring timely access to care and resources.
- Conduct in‑person assessments and home visits in Champaign and Vermilion counties, traveling up to 50% of the time.
- Develop individualized care plans in collaboration with members, families, and interdisciplinary teams.
- Monitor service delivery, track outcomes, and adjust plans to meet evolving needs.
- Serve as the primary point of contact for members, answering questions and resolving issues promptly.
- Maintain accurate documentation in compliance with Centene and regulatory standards.
- Participate in continuous improvement initiatives to enhance member experience.
Qualifications
- Bachelor’s degree in Social Work, Nursing, Healthcare Administration, or related field.
- 2+ years of experience coordinating LTSS or similar health‑care services.
- Strong knowledge of Illinois Medicaid and Medicare regulations.
- Excellent communication, problem‑solving, and interpersonal skills.
- Valid driver’s license and reliable vehicle for travel within Champaign and Vermilion counties.
- Ability to work independently in a remote setting while meeting travel requirements.
Compensation & Benefits
