Key Responsibilities

• Provide comprehensive nursing assessments and develop individualized care plans for patients receiving home health services.
• Serve as the physician’s eyes and ears in the home, monitoring vital signs, medication administration, and wound care.
• Educate patients and families on disease management, medication compliance, and preventive health measures.
• Document all patient interactions accurately in electronic health records and communicate updates to the interdisciplinary team.

Qualifications

• Current, unrestricted RN license in Pennsylvania.
