About This Position
Job Summary
The Arkansas Provider Coalition, LLC is seeking an Enhanced Care Coordinator to join its remote team, preferably based in Northwest Arkansas (Crawford or Sebastian County). This full‑time, non‑exempt role supports individuals with developmental disabilities and behavioral health diagnoses.
Key Responsibilities
- Conduct comprehensive assessments and develop individualized care plans for clients.
- Coordinate medical, therapeutic, and social services across multiple providers.
- Monitor client progress, adjust plans as needed, and document outcomes.
- Serve as the primary point of contact for clients, families, and caregivers.
- Facilitate communication between healthcare teams, community resources, and insurance agencies.
- Ensure compliance with state regulations, HIPAA, and APC policies.
- Participate in regular case review meetings and provide timely reports to the Enhanced Care Supervisor.
Qualifications
- High school diploma or GED required; Associate’s or Bachelor’s degree in health services, social work, or related field preferred.
- Minimum 1‑2 years of experience in care coordination, case management, or a similar role.
- Strong knowledge of developmental disabilities and behavioral health services.
- Excellent communication, organization, and problem‑solving skills.
- Proficient with electronic health record systems and Microsoft Office.
- Valid driver’s license and ability to travel within Arkansas as needed.
Compensation & Benefits
Salary: $32,321.65 annually (fixed). Benefits include health insurance, paid time off, and professional development opportunities.
