Job Summary

Fidelis Care – New Jersey is seeking a Care Manager (RN) to serve our members in Jersey City, Hudson County. This remote/field‑based role combines clinical expertise with community outreach, requiring up to 75% travel for in‑person member visits.

Key Responsibilities

• Conduct comprehensive health assessments and develop individualized care plans for members.
• Coordinate services across medical, behavioral, and social domains to improve health outcomes.
• Perform regular in‑person visits, ensuring compliance with clinical protocols and member satisfaction.
• Document all interactions in the electronic health record system accurately and timely.
• Collaborate with physicians, social workers, and external providers to align resources.
• Educate members and families on disease management, preventive care, and available benefits.
• Monitor and report key performance metrics and quality indicators.

Qualifications

• Active Registered Nurse (RN) license in the state of New Jersey.
• Minimum 2 years of clinical nursing experience, preferably in case management or community health.
• Strong interpersonal and communication skills with a focus on culturally competent care.
• Ability to travel extensively within the local area (up to 75% of work time).
• Proficiency with EHR systems and basic computer applications.
