Job Summary
Fidelis Care – New Jersey is seeking a Care Manager (RN) to provide high‑quality, in‑person care coordination for members throughout Newark, Essex County. This remote‑field hybrid role involves extensive local travel to conduct member visits and support health outcomes.
Key Responsibilities
- Conduct up to 75% travel‑based, face‑to‑face assessments and follow‑up visits with members.
- Develop personalized care plans, monitor progress, and adjust interventions as needed.
- Collaborate with multidisciplinary teams, including physicians, social workers, and community partners.
- Educate members and families on disease management, medication adherence, and preventive health.
- Document all interactions in the electronic health record in compliance with regulatory standards.
- Identify and address barriers to care, connecting members with resources such as transportation, nutrition, and mental‑health services.
- Participate in quality improvement initiatives and report key performance metrics.
Qualifications
- Active Registered Nurse (RN) licensure in New Jersey.
- Minimum 2 years of clinical experience, preferably in case management or population health.
- Strong communication, interpersonal, and problem‑solving skills.
- Ability to travel extensively within Newark and surrounding areas.
- Proficiency with electronic health record systems and Microsoft Office Suite.
