About This Position
Job Summary
Join BHP of Central Ohio as a Care Coordinator / MAT in Newark, Licking County, Ohio. You will coordinate care for adult clients battling substance abuse and mental health challenges, ensuring they receive the right treatment at the right time.
Key Responsibilities
- Assess client needs, develop individualized care plans, and monitor progress.
- Implement and maintain a robust monitoring system to track treatment outcomes.
- Coordinate with clinicians, therapists, and external service providers to deliver integrated care.
- Facilitate medication‑assisted treatment (MAT) protocols and ensure compliance.
- Provide education and support to clients and their families.
- Maintain accurate documentation in line with regulatory standards.
Qualifications & Requirements
- High school diploma or GED required; Bachelor’s degree in Social Work, Psychology, or related field preferred.
- Experience working with adults in substance‑use or mental‑health settings.
- Knowledge of MAT regulations and best practices.
- Strong communication, organizational, and problem‑solving skills.
- Ability to work independently and as part of a multidisciplinary team.
- Valid driver’s license and reliable transportation.
Compensation & Benefits
