About This Position
UnitedHealth Group is hiring a Behavioral Health Case Manager (Re‑entry) in Benton, Marshall County, KY. This full‑time role supports individuals transitioning from correctional facilities back into the community, ensuring they receive the behavioral health services they need.
Key Responsibilities
- Assess the mental health and psychosocial needs of re‑entry clients.
- Develop and coordinate individualized care plans, linking clients to counseling, medication management, and community resources.
- Monitor client progress, adjust interventions, and document outcomes in compliance with HIPAA and company policies.
- Collaborate with correctional staff, social workers, physicians, and local agencies to facilitate seamless transitions.
- Provide crisis intervention and advocacy when needed.
- Maintain accurate case notes, reports, and performance metrics.
Qualifications & Requirements
- Bachelor’s degree in Social Work, Psychology, Counseling, or related field (Master’s preferred).
- Minimum 2 years of case management experience, preferably with re‑entry or behavioral health populations.
- Valid Kentucky licensure or certification (LCSW, LMFT, LCSW‑C, or equivalent) or ability to obtain within 90 days.
- Strong knowledge of community resources in Benton and surrounding areas.
- Excellent communication, empathy, and problem‑solving skills.
- Proficiency with electronic health records and Microsoft Office Suite.
