LTSS Service Care Manager

Carolina Complete Health is hiring a LTSS Service Care Manager in Elizabeth City, Pasquotank County, NC. This full‑time role supports the health and wellbeing of Centene’s 28 million members through long‑term services and supports (LTSS) coordination.

Key Responsibilities

• Assess member needs and develop personalized LTSS care plans aligned with clinical guidelines.

Required Qualifications

• Excellent communication, problem‑solving, and interpersonal skills.
• Proficiency with EHR/CRM software and Microsoft Office Suite.
• Valid NC driver’s license and ability to travel within a 2‑hour radius of Pasquotank County.
