About the Role: Remote Med-Surg RN

As a Remote Registered Nurse specializing in Medical/Surgical care, you will be the clinical cornerstone of our telehealth patient management program. You will conduct comprehensive virtual assessments, develop and monitor individualized care plans, provide patient and family education on disease processes and medications, and collaborate closely with on-site care teams, physicians, and case managers. Your day will involve reviewing patient data from remote monitoring devices, identifying clinical trends, and intervening proactively to prevent hospital readmissions. This is not a triage-only role; it is a deep-dive, relationship-based nursing position that requires sharp assessment skills and the ability to build trust through a screen.

Key Responsibilities

  • Perform detailed virtual health assessments and ongoing evaluations for a panel of adult patients with complex medical-surgical conditions (e.g., CHF, COPD, diabetes, post-operative care).
  • Analyze data from remote patient monitoring (RPM) devices including blood pressure cuffs, glucometers, pulse oximeters, and weight scales.
  • Create, implement, and adjust evidence-based care plans in collaboration with the interdisciplinary team.
  • Provide real-time patient education on medication management, symptom recognition, lifestyle modifications, and wound care.
  • Document all interactions meticulously in the electronic health record (EHR), ensuring compliance with all regulatory and billing standards.
  • Escalate urgent clinical findings immediately to the appropriate on-call provider or emergency services.
  • Participate in daily huddles and weekly case review meetings with the clinical team.

Mandatory Credentials & Experience

  • Active, unencumbered Registered Nurse (RN) license in the United States. Must be eligible to practice in our operational states (we support license compact and will assist with additional state licensure as needed).
  • Bachelor of Science in Nursing (BSN) degree preferred; Associate Degree in Nursing (ADN) with significant relevant experience will be considered.
  • Minimum of 3 years of recent, hands-on Medical/Surgical nursing experience in a hospital or acute care setting. Telehealth experience is a plus but not required for exceptional candidates.
  • Current Basic Life Support (BLS) certification. Advanced Cardiovascular Life Support (ACLS) certification preferred.
  • Proficiency with computers, multiple software platforms, and remote monitoring technology. Excellent digital communication skills.
  • A dedicated, private home workspace that meets HIPAA compliance standards (quiet, secure internet connection).
