About the Role

As a key member of our virtual care team, you will transition your bedside skills to a dynamic digital environment. Your primary mission will be to assess, plan, implement, and evaluate patient care through secure video conferencing, phone calls, and asynchronous messaging platforms. You will manage a panel of patients with acute and chronic conditions, conduct comprehensive health assessments, develop personalized care plans, and provide critical health education and counseling.

Your Day-to-Day Impact

• Conduct initial and follow-up telehealth assessments for patients across a diverse caseload.
• Coordinate care with physicians, specialists, and family members to ensure seamless treatment plans.
• Review and analyze data from remote patient monitoring devices (e.g., blood pressure cuffs, glucometers).
• Provide triage, symptom assessment, and urgent care guidance via our virtual platform.
• Document all interactions meticulously within our electronic health record (EHR) system.
• Participate in interdisciplinary team huddles and continuous quality improvement initiatives.

Requirements

We are looking for a proactive, tech-savvy, and empathetic nurse who thrives in a remote setting. The ideal candidate will possess a blend of clinical excellence and digital communication skills.

Essential Qualifications

• Active, unencumbered Registered Nurse (RN) license in the United States. Multi-state licensure is a strong plus.
• Minimum of 3 years of clinical nursing experience, preferably in primary care, urgent care, or case management.
• Demonstrated proficiency with computers, video conferencing software (Zoom, Teams), and EHR systems.
• Excellent verbal and written communication skills with a patient, compassionate demeanor.
• Ability to work independently, manage time effectively, and maintain a quiet, HIPAA-compliant home workspace.
• Basic Life Support (BLS) certification.

Preferred Attributes

• Certification in Ambulatory Care Nursing (RN-BC) or Telehealth Nursing.

Compensation & Benefits

Salary Range

The estimated salary range for this position is $75,000 – $95,000 USD per year, commensurate with experience, geographic differentials, and multi-state licensure. Final compensation will be determined during the interview process.
