About the Role

As a Remote LVN, you will become a vital part of our interdisciplinary care team, serving patients across the country. Your primary mission will be to conduct initial patient assessments, provide follow-up care, manage chronic conditions, and offer crucial health education—all through secure video conferencing and digital health tools. You will collaborate closely with Registered Nurses, Physicians, and care coordinators to develop and execute personalized care plans, ensuring each patient receives timely, effective, and empathetic support. Your day will involve reviewing patient data, documenting interactions in our electronic health record (EHR), and triaging urgent needs to the appropriate clinical team member.

Key Responsibilities Include:

• Conduct comprehensive virtual patient assessments and vital sign reviews.
• Provide patient education on disease management, medication adherence, and lifestyle modifications.
• Monitor patient-reported outcomes and remote device data (e.g., blood pressure, glucose).
• Triage patient calls and messages, escalating urgent or emergent situations.
• Accurately and promptly document all patient interactions in the EHR.
• Participate in daily team huddles and care coordination meetings.

Requirements

Mandatory Qualifications:

• Active, unencumbered Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license in your state of residence.
• Minimum of 1-2 years of clinical nursing experience, preferably in primary care, case management, or telehealth.
• Proficiency with computers, video conferencing software (e.g., Zoom, Teams), and basic EHR navigation.
• Reliable high-speed internet connection and a dedicated, private workspace.
• Excellent verbal and written communication skills with a patient-centered approach.

Preferred Qualifications:

• Certification in case management (CCM) or as a Diabetes Educator (CDE).
• Previous experience with remote patient monitoring (RPM) programs.
• Bilingual capabilities (Spanish highly desirable).
