About the Role & Your Impact

In this fully remote position, you become a critical frontline connector for our patients. You will manage a dynamic caseload, conducting initial health assessments, providing patient education, and supporting our RN and physician teams in developing care plans. Your day will involve secure video consultations, reviewing patient-reported data, and offering empathetic guidance for chronic condition management, post-acute care, and preventive health. You are not just a nurse; you are a trusted advocate and a cornerstone of our virtual clinic model.

Core Daily Responsibilities

• Conduct comprehensive virtual patient assessments and document findings in our EHR system.
• Perform patient triage, determine urgency, and escalate cases to appropriate providers.
• Provide evidence-based education on medications, diets, wound care, and disease processes.
• Coordinate care with patients’ families, specialists, and primary care providers.
• Administer and monitor treatments within your LVN scope of practice during video visits.
• Maintain strict compliance with HIPAA, state licensing regulations, and telehealth best practices.

What You Bring (Requirements)

We are looking for a proactive, tech-savvy clinician who thrives in an independent remote setting. The ideal candidate is a licensed Vocational Nurse with a passion for telehealth and exceptional communication skills.

Mandatory Qualifications

• Active, unrestricted LVN/LPN license in good standing with your state nursing board. Multi-state licensure is a strong plus.
• Minimum of 2 years of direct clinical patient care experience (medical-surgical, primary care, case management, or home health highly desirable).
• Proven proficiency with computers, video conferencing tools (Zoom, Teams), and EHR systems.
• Dedicated, quiet home workspace with secure, high-speed internet (minimum 50 Mbps).
• Excellent verbal and written communication skills with a patient-first attitude.
• Ability to work a flexible schedule that may include occasional evening or weekend hours to accommodate patient needs.

Preferred Attributes

• Certification in case management (CCM) or telehealth nursing.
• Previous experience in a remote or telehealth nursing role.
• Bilingual abilities, especially Spanish.

Compensation & Benefits Package

Salary Range

$48,000 – $62,000 USD per year
