About the Role: Your Remote Nursing Mission

In this pivotal remote role, you will function as a key member of our clinical care team, bridging the gap between patients and our providers. You will conduct initial patient assessments, manage care coordination, provide health education, and perform essential follow-up—all via secure video and phone calls. You’ll utilize a cutting-edge Electronic Health Record (EHR) system to document meticulously, ensuring seamless continuity of care. Your days will be a dynamic mix of direct patient interaction, clinical task management, and collaborative problem-solving with nurses and doctors, all from your dedicated home workspace.

Core Responsibilities Include:

• Conduct comprehensive patient interviews and vitals assessments via telehealth.
• Triage patient concerns, escalate urgent cases to providers promptly, and provide evidence-based self-care advice.
• Manage patient schedules, coordinate referrals, and arrange for home health or lab services as needed.
• Provide clear, empathetic education on medications, treatments, and disease management.
• Maintain precise, timely, and compliant clinical documentation in the EHR.
• Adhere strictly to all telehealth protocols, HIPAA regulations, and state licensing requirements.

Requirements: Are You Our Perfect Remote LVN?

We need a proactive, tech-savvy, and licensed professional who thrives in an independent yet collaborative remote setting. The ideal candidate is a patient advocate first and foremost.

Mandatory Qualifications:

• Active, unencumbered LVN/LPN license in good standing with your state nursing board. (Multi-state licenses are a plus!).
• Minimum of 2 years of hands-on clinical experience in a fast-paced setting (e.g., primary care, urgent care, home health, or hospital med-surg).
• Demonstrated proficiency with computers, multiple software platforms, and a reliable high-speed internet connection.

• Current BLS certification (ACLS preferred).

Salary Range:
