About the Role

This isn’t a traditional bedside position. We are pioneering a new model of home health support, where our aides leverage technology to provide comprehensive, non-clinical care from a remote setting. You will become the trusted virtual point of contact for clients and their families, ensuring care plans are followed, concerns are addressed promptly, and continuity of care is maintained. Your empathy and organizational skills will bridge the gap between clinical teams and the home environment, all from your dedicated home office.

What You’ll Do: The Remote Impact

Virtual Patient Engagement & Monitoring

Conduct daily or weekly video check-ins to assess client well-being, medication adherence, and safety. You’ll guide clients through using remote patient monitoring (RPM) devices like blood pressure cuffs and glucometers, reviewing data to spot trends and alert clinical staff to potential issues.

Care Coordination & Advocacy

Act as the central coordinator for each client’s care team. You’ll schedule virtual appointments with nurses and therapists, communicate updates from family members, and ensure all documentation in the electronic health record (EHR) is accurate and up-to-date.

Education & Emotional Support

Provide virtual education on disease management, nutrition, and home safety. Offer crucial emotional support and companionship to combat isolation, building strong, trusting relationships with clients and their caregivers through consistent, compassionate communication.

Administrative Excellence

Handle care plan documentation, report writing, and compliance logs with precision. You’ll ensure all remote interactions meet HIPAA standards and company protocols, maintaining the highest level of confidentiality and professionalism.

Compensation & Benefits

We value the critical work you do. For this remote position, we offer a competitive annual salary range of $48,000 – $62,000 USD, commensurate with experience and certifications. Your compensation reflects the specialized skills required for effective remote care coordination.

Requirements

We seek dedicated professionals with a heart for care and the discipline for remote work. The ideal candidate will have:

• Certification: Current Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification.
• Experience: Minimum 1 year of hands-on home health, hospice, or senior care experience.
• Tech Proficiency: Strong command of video conferencing tools (Zoom, Teams), EHR systems, and general computer use. Ability to troubleshoot basic technical issues.
• Remote Work Setup: A private, quiet, and professional home workspace with a reliable high-speed internet connection (minimum 50 Mbps download/10 Mbps upload).
• Soft Skills: Exceptional verbal and written communication, supreme organizational skills, proactive problem-solving, and the ability to work independently with minimal supervision.
• Compliance: Willingness to undergo background check and maintain HIPAA compliance training. Must be a U.S. resident and work from a location within the United States.
