About the Role

In this fully remote position, you will serve as a crucial liaison and support coordinator for our home health patients. You will not perform hands-on clinical or personal care tasks. Instead, you will manage schedules, conduct wellness check-ins via phone or video, assist with medication reminders through our digital platform, coordinate with nurses and therapists, and help patients and families navigate care plans and resources. Your day involves empathetic communication, meticulous documentation in our Electronic Health Record (EHR) system, and proactive problem-solving to address barriers to care. You will become a trusted, familiar voice for our clients, ensuring they feel heard and supported throughout their care journey.

Core Responsibilities

• Conduct scheduled and ad-hoc virtual check-in calls with patients to assess non-clinical needs and provide emotional support.
• Maintain accurate, timely patient records and documentation within our secure EHR system.
• Coordinate and confirm appointments between patients, clinical staff, and community resources.
• Assist patients with understanding and adhering to their care plans and medication schedules through verbal reminders and digital tools.
• Act as a compassionate advocate, identifying social determinants of health needs (like food insecurity or transportation issues) and connecting patients with appropriate support services.
• Participate in daily virtual team huddles and case conferences with the clinical team.

Salary & Benefits
