Key Responsibilities

Your primary mission is to remove barriers to health. You will own the entire patient journey from intake to follow-up.

Core Duties Include:

  • Patient Advocacy & Navigation: Conduct comprehensive assessments, explain medical instructions in plain language, and guide patients through each step of their treatment plan.
  • Care Plan Orchestration: Develop and maintain individualized care plans in collaboration with the clinical team. Schedule and coordinate appointments, procedures, and therapy sessions across multiple providers.
  • Communication Hub: Serve as the main point of contact for patients, families, and providers. Relay critical information, update records meticulously, and facilitate secure team huddles.
  • Resource Connection: Identify and connect patients with community resources, financial assistance programs, and support groups to address social determinants of health.
  • Documentation & Compliance: Maintain accurate, confidential records in our electronic health record (EHR) system. Ensure all activities meet regulatory and payer requirements.
  • Crisis Management: Recognize and escalate urgent patient concerns or potential adverse events to the clinical supervisor immediately.
