Unlike traditional DSP roles, this position focuses on the “coordination of care”—helping clients manage their schedules, navigate healthcare systems, access community resources, and maintain mental wellness through virtual check-ins and strategic planning.

Key Responsibilities

• Conduct daily virtual wellness checks and support sessions via secure video conferencing.
• Coordinate medical appointments and facilitate communication between clients and healthcare providers.

Salary Range
