About the Role

Key Responsibilities

• Conduct comprehensive biopsychosocial assessments via phone and video to identify patient needs, barriers to care, and goals.
• Develop, implement, and regularly update individualized care plans in collaboration with the interdisciplinary care team.
• Proactively schedule appointments, arrange referrals, and coordinate transitions of care (e.g., hospital discharge to home).
• Provide health education on conditions, medications, and self-management strategies to empower patients.
• Act as a relentless advocate for patients, ensuring their voice is heard and their needs are met by the healthcare system.
• Utilize our electronic health record (EHR) and care management platform to document all interactions with precision and timeliness.

Required Experience & Skills

• Education: Active, unencumbered license as a Registered Nurse (RN) or Licensed Practical/Vocational Nurse (LPN/LVN) in your state of residence. A Bachelor’s degree in Nursing (BSN) is strongly preferred.
• Experience: Minimum of 2 years of clinical nursing experience. At least 1 year of direct experience in care coordination, case management, patient navigation, or a related discharge planning role.
• Knowledge: Proficient understanding of chronic disease management, community resources, insurance benefits (Medicare, Medicaid, commercial), and the healthcare continuum.
• Core Skills: Exceptional verbal and written communication, active listening, critical thinking, and cultural competency. Mastery of Microsoft Office Suite and ability to learn new software quickly.

Compensation

We offer a highly competitive salary range of $52,000 – $68,000 USD per year, commensurate with your experience, licensure, and geographic market. This is a exempt, salaried position.
