About the Role

As a Substance Abuse Counselor at Hope Horizons, you will be the frontline professional guiding individuals and families through the challenges of addiction. Working entirely from your home office, you’ll provide evidence‑based counseling, develop individualized treatment plans, and coordinate care with a multidisciplinary team—all via secure video conferencing, phone, and secure messaging platforms.

Core Responsibilities

• Deliver one‑to‑one counseling and group therapy sessions to clients of all ages using CBT, Motivational Interviewing, and relapse‑prevention strategies.
• Conduct comprehensive intake assessments, develop personalized treatment plans, and monitor progress using our electronic health record (EHR) system.
• Collaborate with psychiatrists, medical providers, and social workers to ensure integrated, holistic care.
• Participate in weekly virtual team meetings, case conferences, and continuing education sessions.
• Maintain accurate, HIPAA‑compliant documentation and comply with all regulatory requirements.
• Advocate for clients’ needs and provide crisis intervention when necessary.

Who You Are

• Bachelor’s or Master’s degree in Counseling, Psychology, Social Work, or a related field.
• Licensed or certified counselor (e.g., LCSW, LPC, LCSW‑AD, LMHC) in the state of practice.
• Minimum of 2 years of direct client counseling experience in substance abuse or related behavioral health settings.
• Proficiency with telehealth platforms, electronic health records, and virtual communication tools.

Salary Range
