About the Role
As a Remote Care Coordinator, you will manage a caseload of clients with diverse healthcare needs, acting as their advocate, navigator, and planner. You will work collaboratively with nurses, social workers, physicians, and insurance providers to develop and implement personalized care plans. Your day will involve conducting comprehensive assessments via video and phone, coordinating appointments and services, providing education, and offering crucial emotional support to clients and their families. Key Responsibilities
- Conduct initial and ongoing telephonic/video assessments to evaluate client needs, goals, and social determinants of health.
- Develop, document, and regularly update individualized care plans in collaboration with the clinical team.
- Coordinate and schedule medical appointments, therapy sessions, home health services, and community resources.
- Serve as the primary communication hub for clients, families, and the interdisciplinary care team.
- Provide proactive follow-up to ensure care plan adherence and identify potential issues early.
- Educate clients and families on conditions, medications, and available support systems.
- Maintain meticulous, timely, and accurate documentation in our Electronic Health Record (EHR) system.
- Identify and escalate urgent clinical or psychosocial concerns to the appropriate supervisor or provider.
What You Bring (Requirements)

Required Qualifications
- Associate’s degree in Nursing (ADN), Social Work, Public Health, or a related field. Bachelor’s degree (BSN, BSW, MPH) preferred.
- 2+ years of experience in a care coordination, case management, patient navigation, or social work role within healthcare.
- Active, unrestricted license as a Registered Nurse (RN) or Licensed Social Worker (LSW) is a strong advantage.
- In-depth knowledge of the healthcare continuum, insurance principles (Medicare, Medicaid, commercial), and community resources.
- Proficiency with EHR systems and Microsoft Office Suite.
- Exceptional verbal and written communication skills with the ability to build rapport remotely.
- Strong organizational skills, time management, and the ability to manage a dynamic caseload independently.
- Must reside in the United States and be able to work standard business hours (Eastern or Central Time zones preferred).
Compensation & Benefits
We believe in compensating our team fairly for the vital work they do. This is a full-time, exempt position.
Salary Range
$52,000 – $72,000 USD per year, commensurate with experience, licensure, and geographic market adjustments. Performance-based bonuses are available quarterly.
