Family Support Worker

Position Summary The Social Services Family Support Worker (FSW) is responsible for all clinic psycho-social referrals; screenings and assessments; and short-term interventions. A key component of the position is to coordinate services with medical, dental and behavioral health providers, as well as collaborate with outside service providers.
