About the Role
As a Remote Care Coordinator, you will be the primary point of contact for a assigned panel of patients, coordinating appointments, managing care plans, and ensuring communication flows smoothly between doctors, specialists, therapists, and family members. Key Responsibilities
- Conduct comprehensive patient assessments via phone and video to understand medical, psychosocial, and environmental needs.
- Develop, implement, and regularly update individualized care plans in collaboration with patients, families, and the interdisciplinary care team.
- Schedule and coordinate all medical appointments, procedures, and follow-up care, ensuring patients have clear instructions and pre-visit preparation.
- Proactively monitor patient progress, identify potential issues or red flags, and intervene to prevent complications.

What We’re Looking For (Requirements)
- Education: Associate’s degree in Nursing, Health Administration, Social Work, or a related field. Bachelor’s degree preferred.
- Experience: 2+ years of experience in a care coordination, case management, patient navigation, or clinical liaison role within healthcare. Experience with chronic disease management (e.g., diabetes, heart failure, COPD) is a significant plus.
- Skills: Exceptional verbal and written communication skills. Superior organizational and time-management abilities with a talent for prioritizing tasks. Proficiency with EHR systems (e.g., Epic, Cerner) and Microsoft Office Suite. Strong problem-solving and critical thinking skills.
- Mindset: Deep empathy, patience, and resilience. A proactive, patient-centered approach. Ability to work independently in a remote setting with high self-motivation and discipline.
- Certification: Certified Case Manager (CCM) or similar certification is highly desirable but not required.
