About the Role & Your Impact

In this fully remote position, you will become a cornerstone of our patient support team. You will conduct virtual check-ins, oversee care plans, educate patients and families on health management, and coordinate with in-field nurses and therapists. Your primary mission is to ensure patients feel heard, understood, and empowered to adhere to their treatment regimens, all while working from the comfort and safety of your own home. You will manage a caseload of patients, using secure video calls, phone, and messaging platforms to provide consistent, compassionate support.

Key Responsibilities

• Virtual Patient Monitoring: Conduct daily or weekly telehealth sessions to assess patient well-being, symptoms, and adherence to medication and therapy plans.
• Care Coordination: Act as the central communication hub between patients, their families, doctors, and on-site home health nurses to ensure seamless care delivery.
• Patient Education & Advocacy: Teach patients and caregivers about disease management, nutrition, safety, and use of medical equipment. Advocate for patient needs with the clinical team.
• Documentation & Reporting: Meticulously document all interactions, assessments, and communications in our electronic health record (EHR) system in real-time.
• Emotional Support: Provide a listening ear, emotional reassurance, and psychosocial support to combat isolation and anxiety in chronic care management.
• Emergency Protocol: Identify red-flag symptoms and escalate immediately to nursing supervisors or emergency services according to strict protocols.

What We’re Looking For (Requirements)

This role requires a unique blend of healthcare certification and tech-savvy communication skills. The ideal candidate is a self-starter with a nurturing demeanor and impeccable organizational skills.

Mandatory Qualifications

• Active Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification in your state of residence.
• Minimum of 1 year of hands-on home health, hospice, or skilled nursing facility experience.
• Reliable high-speed internet, a dedicated quiet workspace, and a computer with a webcam and microphone.
• Proficiency with computers, video conferencing tools (Zoom, Teams), and willingness to learn new EHR software.
• Excellent verbal and written English communication skills.

Preferred Attributes

• Experience with telehealth platforms and remote patient monitoring.
• Bilingual or multilingual abilities (Spanish, Mandarin, etc.) are a significant plus.
• Demonstrated empathy, patience, and ability to build rapport without physical presence.

Compensation & Benefits

Salary Range

$38,000 – $52,000 USD per year, commensurate with experience, certification, and geographic cost-of-living adjustments. Overtime eligibility as per state and federal law for non-exempt roles.
