  • Patient Education & Advocacy: Teach patients and caregivers about disease management, nutrition, safety, and use of medical equipment. Advocate for patient needs with the clinical team.
  • Documentation & Reporting: Meticulously document all interactions, assessments, and communications in our electronic health record (EHR) system in real-time.
  • Emotional Support: Provide a listening ear, emotional reassurance, and psychosocial support to combat isolation and anxiety in chronic care management.

What We’re Looking For (Requirements)

Mandatory Qualifications
