About the Role

What You Will Do

• Conduct daily virtual check-ins with assigned clients via phone or video call to assess well-being, mood, and safety

• Coordinate with nurses, therapists, and social workers to adjust care plans as needed

Requirements

• Valid Home Health Aide (HHA) certification from an accredited program (state-certified or NNAAP-compliant)
