About the Role

As a Physical Therapist at HealthFirst Rehabilitation, you will be at the heart of our mission to improve patient outcomes through evidence-based practice. You’ll assess, diagnose, and treat individuals suffering from musculoskeletal, neurological, and cardiopulmonary conditions. Your expertise will be crucial in developing personalized rehabilitation plans, guiding patients through therapeutic exercises, and utilizing advanced modalities to facilitate recovery. This role offers the opportunity to work with a diverse patient population across various settings, including outpatient clinics, home health, and potentially community outreach programs. Key Responsibilities

• Conduct thorough initial evaluations and ongoing assessments of patients’ physical function and mobility limitations.
• Develop, implement, and modify individualized treatment plans based on patient goals and clinical findings.
• Utilize a wide range of therapeutic techniques, including therapeutic exercises, manual therapy, modalities (e.g., ultrasound, electrical stimulation), and patient education.
• Provide direct patient care in various settings, ensuring safety, efficacy, and adherence to best practices.
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient care.
• Document patient progress meticulously and communicate findings clearly.

Requirements

To excel in this role, you must possess the following qualifications:

• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current licensure as a Physical Therapist in the state of California.
• Strong clinical skills in musculoskeletal, neurological, and cardiopulmonary rehabilitation.
• Excellent communication and interpersonal skills to build rapport with patients and families.
• Ability to work collaboratively within a multidisciplinary team environment.
• Proven experience in outpatient physical therapy practice preferred.
• Commitment to continuing education and professional development.
