About the Role

Salary Range

Key Responsibilities

  • Conduct regular virtual check-ins and video sessions with clients to assess their physical, emotional, and social needs, providing real-time support and crisis intervention when necessary.
  • Assist clients with planning and executing daily living activities such as meal preparation, medication management, personal hygiene, and transportation arrangements.
  • Collaborate with family members, onsite caregivers, and healthcare professionals through virtual meetings to coordinate care plans and ensure consistency.
  • Maintain up-to-date digital records using our secure electronic health record (EHR) system, documenting client progress, incidents, and service delivery.
  • Develop individualized support plans that incorporate clients’ goals, preferences, and assistive technologies, and regularly review and adjust them as needed.
  • Promote community inclusion by facilitating virtual social activities, connecting clients with online resources, and encouraging participation in remote recreational programs.
  • Advocate for clients’ rights and empowerment, ensuring they have access to necessary services and are treated with dignity and respect.
  • Participate in ongoing training and quality improvement initiatives to enhance your skills and stay current with best practices in remote support.

Requirements

  • High school diploma or GED required; an associate’s or bachelor’s degree in psychology, social work, human services, or a related field is highly preferred.
  • At least 1 year of professional experience in direct support, caregiving, or a human services role, with demonstrated ability to work independently and compassionately.
  • Proficiency with computers, video conferencing platforms (e.g., Zoom, Microsoft Teams), and electronic health record systems; must be able to troubleshoot basic technical issues.
  • Excellent communication skills, both written and verbal, with the ability to build trusting relationships with clients remotely.
  • A dedicated, quiet home office space with a reliable high-speed internet connection (minimum 25 Mbps upload/download).
  • Valid driver’s license and personal vehicle with valid insurance; occasional in-person visits to clients’ homes or medical appointments may be required (travel expenses reimbursed).
  • Certification as a Direct Support Professional (DSP) from a recognized accrediting body, or commitment to complete certification within the first 6 months of employment.
