About the Role

As a Direct Support Professional at Community Care Solutions, you will be the vital link between individuals we serve and their community. Your primary responsibility is to implement individualized support plans designed to promote personal growth, skill development, and community integration. This involves providing hands-on assistance with daily living activities, facilitating social and recreational opportunities, ensuring a safe and nurturing environment, and actively participating in team meetings and planning sessions. You will work closely with a team of professionals and the individuals and their families to achieve shared goals, making a profound difference in their daily lives and long-term well-being.

Key Responsibilities

  • Implement personalized support plans developed by the team and individuals served.
  • Provide hands-on assistance with activities of daily living (ADLs) including personal care, meal preparation, and household tasks.
  • Facilitate community involvement through transportation, recreational activities, and social events.
  • Maintain a safe, clean, and supportive living environment.
  • Document services provided accurately and timely, contributing to effective team communication.
  • Actively participate in team meetings, staffings, and training sessions.

Requirements

Success in this role requires a unique blend of personal qualities and professional qualifications. We seek individuals who are:

  • Compassionate, patient, and empathetic, with a genuine passion for supporting individuals with disabilities.
  • Reliable, punctual, and able to work independently while being a team player.
  • Possess a High School Diploma or equivalent (GED).
  • Have at least 6 months of verifiable experience providing direct support to individuals with disabilities (residential, day program, or similar settings).
  • Valid driver's license and reliable transportation (required for community outings).
  • Current CPR/First Aid certification (or willingness to obtain within 90 days).
  • Strong communication and interpersonal skills.

Additional qualifications such as a Certificate in Human Services, Developmental Services, or related field, or experience working with specific populations (e.g., autism, intellectual disabilities), are a plus.
