The Service Coordinator Supervisor at Higher Ground IHS in Albany, Albany County, oversees a team of Service Coordinators delivering person‑centered support to individuals with disabilities and seniors. Key Responsibilities
- Supervise, coach, and evaluate Service Coordinators to maintain high‑quality service delivery.
- Ensure all service plans, coordination activities, and documentation meet NYS DOH requirements.
- Facilitate interdisciplinary team meetings and case reviews to support client goals.
- Monitor performance metrics, identify improvement opportunities, and implement corrective actions.
- Provide training on person‑centered practices, self‑determination, and regulatory updates.
- Collaborate with external agencies, healthcare providers, and community partners.
- Manage scheduling, workload distribution, and resource allocation for the coordination team.
- Prepare reports, audits, and compliance documentation for senior management.
Qualifications & Requirements
- Bachelor’s degree in Social Services, Human Services, Healthcare Administration, or related field (Master’s preferred).
- Minimum 3 years of experience in service coordination for disability or senior services.
- At least 2 years of supervisory or team‑lead experience.
- Strong knowledge of New York State Department of Health regulations and Medicaid/HCBS guidelines.
- Excellent communication, interpersonal, and conflict‑resolution skills.
- Proficient with electronic health records and case management software.
- Ability to travel locally within Albany County as needed.
Compensation & Benefits
Salary range: $67,909.47 annually.
